Cigna, Multi-Plan - PPO, United Health Care (HMO, plus all Commercial Health Plans)  VERIFY HERE:
 Lactation services are covered under the Affordable Health Care Act & families often receive services with
no copay or deductible. Plans vary in copay/coinsurance fees. Check with your provider for specific details. 
Our Fee is paid at the Lactation Visit when not 'pre-approved' before your planned appointment,
We provide an insurance superbill at our appointment that you file with your insurance for reimbursement.
